At Canyon Family Medicine, we offer a subscription membership model to help manage health care costs for those who are uninsured. Direct Primary Care includes all of your routine health maintenance, health promotion, disease prevention, counseling, patient education, diagnosis, and treatment of acute and chronic illnesses. We offer a full scope of Primary Care services included with our Direct Primary Care Membership. In addition to all of this, we offer discounted rates for labs and radiology testing.


At Canyon Family Medicine, our focus is not just on getting you healthy, it’s on KEEPING YOU HEALTHY for a lifetime! The best way, we have found, to support our patients in their goal to say well is to offer our care on a “subscription” basis. Our membership subscription allows patients the opportunity to benefit from all that we have to offer without having to be nickel-and-dimed for every itemized service or product. Other benefits of membership include the fact that we keep ancillary costs to a minimum and specialty services.

Our Membership is affordable for the entire family and gives you and your loved ones access to primary care and walk-In visits when and where you need it. We have the time to spend getting to know our patients because we limit the number of active members in our practice. This means that you will always get the attention you deserve at Canyon Family Medicine – whether it’s here in our office, talking with your doctor via our telemedicine platform, or even in the comfort of your home.

We know that you will love all of the benefits of membership, but we also know that it can be scary to sign a contract. Don’t fear. You can cancel your membership at any time!


Monthly membership includes all of your primary care services including unlimited routine & urgent care, annual physical exams, minor procedures, on-site labs, EKG’s and other on-site testing.

Your first payment is a combination of the $75 enrollment fee per adult plus the first month’s membership fee. The enrollment, plus membership fee is processed immediately upon membership approval. The enrollment fee is non-refundable. An enrollment fee is $75 per person, with no enrollment fee for children with a family max of $125.

Example :

  • 1 Adult = $75 enrollment fee + $95 monthly membership fee = $170
  • 1 Adult + 1 child = $75 + $95 + $20= $190
  • 2 Adults = $125 enrollment fee + $95 + $95= $315
  • 2 Adults + 2 children = $125 enrollment fee + $230 = $355
  • 2 Adults + 3 children = $125 enrollment fee + $240= $365
  • Max membership fee per family is $240 per month

The monthly membership fees are $95 per adult (ages 19 and up ) and $20 per child (ages 0 to 18). Max membership fee per family is $240 per month. The fees are paid by the calendar month on an ongoing basis until cancellation. Payment is required to remain an active member and receive service.

If membership fees are paid one year in advance get one month free or your vitamin B12 weight loss shots are on us,

ALL members must enroll Auto-Pay of monthly invoices with a Bank ACH, Credit Card, or Debit Card. We don’t accept American Express. Due to our low staff and low overhead business model, we cannot invoice or bill membership or other fees.

Memberships require 3 MONTH TERM CONTRACT TO START, cancellation and re-enrollment will require a fee, as noted below.


We understand that your needs may change, and we want to help you through whatever transitions you’re making. If you need to cancel, we would appreciate a 30-day notice. You may cancel your membership at any time after the 3-Month Membership Contract minimum has been met for any reason by written message or email and will be processed the following business day. All charges, including membership fees, will continue to accrue until the notice of cancellation is received. You will be required to pay the current balance for the current month and charged the remaining outstanding balance at the time of cancellation. If you paid for an annual membership fee and decide to cancel, your remaining balance will be refunded without a hassle.


If a member discontinues membership – by choice or lack of payment – the member may rejoin in the future at the discretion of the practice but will be required to pay a Re-Enrollment fee of $75 per adult (maximum of $125 per family) as long as practice capacity hasn’t been reached.

Enrollment is subject to availability. Canyon Family Medicine reserves the right to add or decrease any fee, at any time, at the sole discretion of the practice and decline future re-enrollment.
